Fistulated Hydatid Abscess [march 2007]

INFORMATION

A 78 year old patient presented with continuous pain in the right hypochondrium, with the appearance of a palpable mass and spiking fevers of up to 40ºC.  

In image 1 the tumour is seen located in the last intercostal spaces, anterior axillary line. It had a firm consistency and had bright red coloring with a yellowish zone and central fluctuation. In the center of the tumour was an area of punctiform erosion (fistula orifice).  

In image 2 a tumour is seen in the liver with a low density and homogeneous content, well delineated by a capsule and with calcification of the wall. Swelling of the soft tissues is clearly seen, with loss of separation between the muscular planes and tumour infiltration of the skin with subcutaneous weaving.  

This was a case of a hydatid cyst with abscess, fistulating to the skin through the subcutaneous and muscular planes.
