| A 83 year
old male admitted for another reason presented
with several days of episodes of left-sided abdominal
colic, accompanied by intense borborigmi. During
the episodes a mass of soft consistency was palpated
in the left abdomen, painful to palpation, although
without signs of peritoneal irritation. The simple
abdominal x-ray did not show anything of interest.
The abdominal CT showed herniation of the colon
(arrows) through the Spigelian fascia (tip of
arrow), situated between the aponeurosis of the
abdominal transverse muscle and the lateral border
of the rectus anterior.
In between crises the patient was found well.
The Spigelian hernia can be difficult to diagnose.
