A 53-year-old man complaining of right flank pain
that had begun gradually 20 minutes earlier was
admitted in our hospital. By the time of admission
the pain had increased and spread downwards. Narcotics
were required for the patient’s relief.
While he was at rest, in the emergency department,
experienced a new bout of hypogastric colicky
pain along with urgency and dysuria which alleviated
after the expulsion with urine of several soft
masses with the appearance of skin grape-fruit
(panel 1A, B). A CT of abdomen showed a large
hydatid cyst in the right kidney (panel 2) with
several small vesicles arising from the inner
surface of the wall cyst (endogenous vesication,
arrowheads). The cyst was surgically removed and
treated with an oral course of albendazole. Five
years later the patient is asymptomatic with no
further evidence of recurrece.
